Here are the steps you would
take
to initiate our General Clinical
Medical Practice Service as provided by MDC:
1) Call Dr. Gates 678.591.6509 to request a general health evaluation. After you have given Dr. Gates a general verbal consent to provide medical care to you, and you have acknowledged the privacy of the conversation between you (the patient) and Dr. Gates, Dr. Gates will perform a preliminary medical assessment of your needs for further medical evaluation.
2) Based on Dr. Gates' preliminary medical assessment or mobile care evaluation, Dr. Gates will advise you with regards to appropriate medical laboratory testing. Various patient-directed laboratory testing facilities are available regionally and nationally, for our online patients to access. You might also consider visiting us at our practice location in Atlanta on Cleveland Avenue, for direct phlebotomy service or for diagnostic procedures such as skin biopsy with frozen-section (an immediate diagnostic procedure), fine-needle aspiration of lumps, pap-smears, and blood smears. The results from patient-direct access testing facility can be sent to Medical Diagnostic Choices, Atlanta, Georgia, for interpretation by Dr. Gates. You will be given a full assessment and plan of action for further diagnostic management and/or therapeutic care. For mobile medical care, our physician will come to assess your medical needs in the privacy of your home, and collect phlebotomy samples for further medical laboratory testing.
3) After consenting to allow Medical Diagnostic Choices to provide primary care and laboratory medical service to you, further instructions will be given to you at the time of initiating this patient-physician agreement. You may send your demographics and complete address, contact number, as well as other pertinent information to the address indicated above.
If you desire, a final medical evaluation report by Dr. Gates will be confidentially available to you in detailed simple, plain ENGLISH, as well as recommendation for further clinical evaluation and follow-up.
